Eligibility

The C2C Locums Provider Referral Bonus Program ("Program") is intended to reward individuals who refer qualified physicians and advanced practice providers (“APP”, i.e., Nurse Practitioners, Physician Assistants, and CRNAs) to C2C Locums, LLC ("C2C Locums").

The referral bonus is intended solely as compensation for introducing a qualified healthcare professional to C2C Locums and is not compensation for patient referrals, healthcare referrals, or the generation of federally reimbursable business.

Qualified Referral

A Qualified Referral is a physician or APP who:

• Is legally authorized to work in the United States.
• Meets C2C Locums’ and C2C Locums’ clients’ credentialing standards.
• Has not previously been employed by, contracted with, interviewed by, or been in active discussions with C2C Locums within the preceding twelve (12) months.
• Is not already identified in C2C Locums' recruiting database before the referral is received. C2C Locums’ database is confidential and will not be shared with any third party under any circumstances.
• Is referred using C2C Locums’ official referral submission process.
• C2C Locums has sole discretion as to whether or not to contract with or otherwise submit a referred provider as a candidate for a potential assignment and may refuse or otherwise withdraw a referred provider at any time and for any reason at its sole discretion and without liability whatsoever.

Referral Bonus & Conditions

A one-time referral bonus of Seven Hundred Fifty U.S. Dollars ($750.00) will be paid only after all of the following conditions have been satisfied:

• The referred provider signs an Independent Provider Agreement with C2C Locums;
• The referred provider successfully completes credentialing and onboarding;
  • Both internally (C2C Locums credentialing) and external hospital privileges (if applicable);
• In addition to the other conditions set forth herein, the referring party’s eligibility for the Referral Bonus is contingent upon the referred provider candidate submitting a completed C2C Locums application within sixty (60) days of the application being provided to the referred provider by C2C Locums (the date C2C Locums receives a complete application being the “Application Completion Date”) and commencing a clinical assignment with a C2C Locums client within 365 days of the Application Completion Date.
• All required invoices and documentation have been processed and C2C Locums has received payment from a client for such provider’s service.

If you qualify to receive a referral bonus while you are on assignment with C2C Locums, you will receive your bonus automatically through your current payroll. If you qualify for the bonus but are not on an assignment at that time, you will be contacted by C2C Locums to confirm payment instructions. Only one referral bonus will be paid for each referred provider. Bonuses are processed and paid out within 60 days of receipt by C2C Locums of payment from the applicable client (i.e., healthcare care facility).

First Referral Rule

If multiple individuals refer the same provider, only the first referral received and documented by C2C Locums will be eligible for the referral bonus. C2C Locums’ confidential records shall be final in determining referral eligibility. Multiple bonuses will not be paid for the same referred individual, and the bonus will be awarded to the person listed on the form or application as the referring individual. A complete name, address, email, and phone number are required for the referring individual to be eligible.

Compliance With Healthcare Laws

This Program is intended to comply with all applicable federal and state laws.

Specifically:

• The referral bonus (and the amount thereof) is not based upon the volume or value of patient referrals, designated health services, Medicare or Medicaid business, any other federally reimbursable healthcare program business or any other business generated from the referral.
• The referral bonus is paid (subject to the conditions set forth above) solely for identifying and introducing a qualified healthcare professional to C2C Locums that ultimately works on assignment procured via C2C Locums for which C2C Locums receives payment.
• By participating in this Program, you agree that you shall not refer patients, healthcare services, or business to C2C Locums or any healthcare facility.
• Nothing in this Program shall be construed as an inducement to influence clinical decision-making or patient referrals. These principles align with the general requirements underlying the Stark Law and Anti-Kickback Statute.

HIPAA and Privacy

You, the referring individual, represent and warrant that you personally know the provider and have the referred provider’s permission to provide their contact information to C2C Locums for the purpose of contacting them regarding possible locum tenens opportunities and related services.

Referrals should include only contact information reasonably necessary for recruitment purposes. Specifically, please provide the name, specialty, phone number and email address of the referred provider. You can fill out the referral form at https://c2clocums.com/refer.html.

No protected health information (PHI), confidential patient information or other personal information should be submitted as part of a referral.

Taxes

Referral bonus payments may constitute taxable income. Recipients are solely responsible for any applicable federal, state, or local taxes. C2C Locums reserves the right to issue or require any required IRS tax reporting forms C2C Locums may deem necessary or appropriate and recipient agrees to comply with any requests from C2C Locums. Recipient shall indemnify, defend, and hold harmless C2C Locums and its affiliates from and against any and all tax obligations, liabilities, penalties, interest, claims, costs, and expenses arising from or related to any referral bonus payment. In order for nonemployees to receive payment, they must submit a completed W-9 at the time the referral is made and will receive a Form 1099-MISC reporting the referral payment as taxable income by January 31 of the following year. Unless otherwise required by applicable law, there will be no tax withholding on these payments, and the recipient is responsible and shall indemnify C2C Locums for his or her own taxes arising from the payment.

Exclusions

The following individuals are not eligible unless otherwise approved by C2C Locums in writing:

• Employees, owners, officers, executives or any of their “family members” of C2C Locums or C2C MSO, LLC, a management services organization. C2C Locums defines “family member” as those immediate family members who live in the home of the staff member — specifically, spouse, children (natural, adoption, or under legal guardianship), stepchildren, and dependents under the age of 24.
• Individuals prohibited by law or employer policy from accepting referral compensation;
• Providers excluded from participation in federal healthcare programs (e.g., OIG/SAM or state or federal Medicaid).
• In a case of "reciprocal referring" (two new candidates applying and referring each other), subject to all terms and conditions set forth in this Program, C2C Locums will pay the referring party $500.00 (rather than $750.00). Providers cannot refer themselves.

Right to Modify

C2C Locums reserves the right to modify, suspend, or terminate this Program without prior notice at any time and for any reason. All eligibility determinations shall be made solely by C2C Locums and are final. By submitting a referral via the referral form you are agreeing to abide by the terms and conditions set forth in this Provider Referral Bonus Program.
